Intuitive Surgical puts a number on its instrument price cuts: 3% to 5% per procedure

Intuitive expects instrument price cuts to take 3% to 5% off da Vinci instrument revenue per procedure by the end of 2027, as US growth slows and an underlying margin above its own range leaves room to pay for it.

Intuitive Surgical sells the da Vinci surgical robot, but most of its revenue comes after the sale, from the instruments and accessories consumed in each operation: about 59% of revenue in the September 2026 quarter, calculated from the reported figures. With those results the company said its planned instrument price cuts would lower da Vinci instrument revenue per procedure by about 3% to 5% once fully in place, measured with procedure, product and country mix held constant. On its previous quarterly call management had declined to size the same plan. It set the new estimate beside the 7% it had expected when it launched its first extended-use program.

The quarter's reported profit makes that harder to see. GAAP operating margin, operating profit as a share of revenue, fell to 19.4% last quarter, well below the quarter before. The cause was a $450 million charge to settle an antitrust class action over da Vinci, agreed without any admission of liability and still subject to court approval. With the itemised litigation charges added back, the GAAP margin would have been 34.4%, calculated from the reported figures. On the company's non-GAAP measure, which also leaves out stock-based pay, it was 41.6%.

The cuts arrive in two pieces. In China, a government pricing policy brings a sharp fall in instrument prices from November. Elsewhere, Intuitive will sell instruments rated for more uses and introduce volume-based pricing, one country at a time. The US launch waits on a clearance from the Food and Drug Administration that is still pending; Europe follows later, and the chief financial officer, Jamie Samath, said the full effect would not be reached until the end of 2027. The reduction therefore builds through the year instead of landing all at once.

Revenue per procedure has been rising, which is what lets the company give some back. Da Vinci instrument revenue per procedure was about $1,860 last quarter, higher than a year earlier, lifted by single-port procedures and instruments made only for the newest da Vinci 5. Management said those mix gains, along with growing use of force-feedback instruments, sit outside the estimated cut and will keep pushing the figure the other way. How the two net out depends on how quickly the newer products spread.

The timing matters because growth in the company's largest market has slowed. US da Vinci procedures grew 10% last quarter, the slowest of this year's three quarters. Management said that matched what it had expected and pointed to a demanding comparison with 16% growth a year earlier; it is also watching lower enrolment in Affordable Care Act plans and changes to Medicaid eligibility. Hospitals are still buying robots, with system placements up by more than a quarter on a year earlier, and the company narrowed its full-year guidance range for worldwide da Vinci procedure growth.

Year-on-year growth in procedures, as the company reported it on its quarterly calls.

The company said the cuts are concentrated in high-volume benign operations, such as hernia repair and gallbladder removal, where cost can stand in the way of adoption, particularly outside the US and in ambulatory settings, though management also counted them among the supports for US growth. Those procedures are already growing quickly abroad: outside the US, benign da Vinci procedures rose 41% last quarter, faster than in either earlier quarter of the year. Samath described the cuts as "a deliberate choice to broaden adoption in high-volume benign procedures".

Whether that pays off depends on elasticity, how many extra procedures a lower price brings in. Samath said Intuitive believes the actions will drive elasticity, citing the company's own review of the 2020 program, and added that any volume gain would build over time and would not switch on straight away. For now that is the company's reading of its own data.

The margin gives Intuitive room to try. Samath restated that management expects operating margin to sit between 35% and 40%, said the company has capacity to invest from where it is now, and expects the gross-margin hit from pricing to be modest and offset by cost reductions next year. The cash cushion is thinner than it was: the company spent $2.94 billion on share buybacks last quarter, and the smaller cash pile led it to cut its full-year forecast for other income. Samath said the company would set out its spending plans with next year's guidance in January.
